Birthday Party Guest Waiver

Printable Nordic Soccer guest waiver for party guests. Each child who plays needs one signed by a parent, with medical info and an emergency contact.

Nordic Soccer helps children grow through thoughtful Scandinavian-style coaching, age-appropriate instruction, and joyful play. Families across Los Angeles and the San Fernando Valley can compare programs, find neighborhood schedules, meet our coaches, and choose the right next step for their player. Every program is designed around development, belonging, and a lasting love of the game.
